How Does ERP Therapy Work?

Obsessions and compulsions keep clients stuck in a loop:


Anxiety → Compulsion → Temporary Relief → Increased Anxiety Over Time

ERP interrupts this cycle.
During ERP therapy, the client practices exposure—intentionally approaching a feared thought, feeling, or trigger—and then practices response prevention, meaning they resist the compulsion that normally provides temporary relief.

Examples include:

  • Touching something perceived as “contaminated” and not washing hands

  • Driving past a feared location without excessive checking or reassurance

  • Having an intrusive thought without engaging in mental rituals

  • Leaving objects “imperfect” without rearranging or fixing them

  • Allowing uncertainty without seeking reassurance from others

Through repeated practice, the brain learns a new, healthier pattern:


Anxiety rises → You stay with it → Anxiety falls on its own → Confidence grows

Your first session involves meeting with an ERP-trained clinician who will help you identify your intrusive thoughts, triggers, compulsions, and the ways your anxiety is impacting daily life. Together, you’ll map out the OCD or anxiety cycle unique to you.

Then, you’ll collaboratively build a hierarchy—a step-by-step list of exposures ranging from least to most challenging. This ensures the process feels safe, predictable, and paced appropriately.

  • Identifying compulsions, rituals, or reassurance habits

  • Creating personalized, structured exposure exercises
